These reviews are designed to be a comprehensive check-in. They allow our clinical team to monitor your condition, discuss any symptoms you may be experiencing, and ensure that your current medication or treatment plan is working as well as it possibly can. It is also an ideal time for you to ask questions and set personal goals for your wellbeing.
๐ How to book your review
If you have not yet booked your long-term condition annual review, we encourage you to contact your usual reception team at your local surgery branch. They will be happy to find a convenient time for you to come in and speak with one of our clinicians.
๐ Preparing for your appointment
To make the most of your time with us, some patients may need to do a little bit of preparation at home. If you are normally asked to provide blood pressure readings as part of your review process, please follow these steps:
๐ Complete 5 full days of readings before your scheduled appointment.
โ๏ธ Take two readings in the morning and two readings in the evening (2x AM and 2x PM).
๐ Record these clearly and remember to bring the completed 5-day log with you to the surgery.
Providing these home readings allows our clinicians to see a clearer, more accurate picture of your health over time, rather than just a single snapshot taken on the day of your visit.
