Do you serve my permanent residential address?

You can check that you live within the catchment area we provide services for here Find a GP – NHS (www.nhs.uk). To save you time, please note we will not register new patients who do not live within this catchment area.

Can I fill in a paper form?

Yes – but only if you cannot use the NHS online services.

You may collect a paper version of the form from our reception team during opening hours. You will then need to fill in the form and return it to reception for processing. We aim to complete paper registrations within 2 weeks.

Can I access my information online?

Yes – please bring 2 types of ID with you if you can’t use the NHS App and would also like to register for online access directly.
